The textbook offers a roadmap for collaboration between the fields of primary care and public health professionals. Case studies, practical recommendations, data resources, and commentaries from national leaders in practice and academia are included in the textbook to help professionals improve population health and reduce health care costs. The Practical Playbook will also be holding a first national meeting in May 2016, “Bringing Public Health and Primary Care Together.” The meeting will bring together leaders and practitioners from across health sectors to collaborate on ways to improve population health.
