CMS selects 14 regions for new primary care model

Comprehensive Primary Care Plus (CPC+) is the only medical home model that is specifically identified in the Quality Payment Program under the Medicare Access and CHIP Reauthorization Act (MACRA) as an advanced Alternative Payment Model (APM).


Comprehensive Primary Care Plus (CPC+), the CMS Innovation Center's new national advanced primary care medical home model, is now accepting applications. The deadline to apply is Sept. 15.

CPC+, a multipayer program that will include 5,000 primary care practices, begins in January 2017. CPC+ is an opportunity for primary care practices that have been working to implement the patient-centered medical home model to receive additional payment outside of the fee-for-service system for providing comprehensive, high-quality care.

CPC+ offers 2 primary care practice “tracks” with incrementally advanced care delivery requirements and payment options to meet the diverse needs of primary care practices. The regions selected are:

  • Arkansas: Statewide
  • Colorado: Statewide
  • Hawaii: Statewide
  • Kansas and Missouri: Greater Kansas City region
  • Michigan: Statewide
  • Montana: Statewide
  • New Jersey: Statewide
  • New York: North Hudson-Capital region
  • Ohio: Statewide and Northern Kentucky (Ohio and Northern Kentucky region)
  • Oklahoma: Statewide
  • Oregon: Statewide
  • Pennsylvania: Greater Philadelphia region
  • Rhode Island: Statewide
  • Tennessee: Statewide

CPC+ is the only medical home model that is specifically identified in the Quality Payment Program under the Medicare Access and CHIP Reauthorization Act (MACRA) as an advanced Alternative Payment Model (APM). Most practices that qualify for CPC+ will receive significant additional payment and, by qualifying as an advanced APM, will be excluded from the Merit-Based Incentive Payment System (MIPS).
