New cardiovascular disease risk reduction resource available

The Million Hearts® initiative recently announced a new Cardiovascular Disease Risk Reduction Model to calculate individualized heart attack and stroke risk scores for patients.

This model uses a data-driven approach to lowering cardiovascular risk across the population and proposes a new way to pay for prevention. Under the program, physicians screen eligible Medicare beneficiaries to determine the patients' individualized risk for heart attack or stroke in the next 10 years and will receive a monthly value-based payment for lowering risk across their patient panel.

The Million Hearts® initiative was launched in 2011 to prevent 1 million heart attacks and strokes by 2017. ACP is a partner in the initiative, which is co-led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS).
