ACP Quality Connect offers diabetes webinar series

ACP Quality Connect is hosting a series of webinars from April through June 2015 to help physicians implement quality improvement strategies to improve care of their diabetic patients.

The first webinar will be April 7 from 7 to 8 p.m. EST. It will be led by quality improvement expert Doron Schneider, MD, FACP, chief patient safety and quality officer at Abington Health in Abington, Pa. Learn how to recapture the joy in your work as a clinician through a renewed focus on your patients, your team, and your results.

After attending the session, participants will be able to:

• Describe the quality landscape, including team-based care, the Chronic Care Model, and Patient-Centered Medical Homes
• Identify the root cause of gaps
• Create a PDSA and run chart
• Engage front line staff to improve quality
