Free tools are available for Guided Care, a new model of primary care for older adults with chronic conditions and complex health needs that is designed to improve quality of care and efficient resource use.

The model, developed by researchers at Johns Hopkins, includes a registered nurse based in the practice who works in partnership with several primary care physicians to provide coordinated, patient-centered, cost-effective care to 50 to 60 of their chronically ill patients.
